During an investigation it was determined personal use of cell phones by caregivers while supervising children.

Annual fire inspection was not available for review at time of inspection.

During time of inspection the electronic child safety alarm was not working on bus 1.

Six out of ten current immunization records were not available to review at time of inspection.

A caregiver used poor judgement when she spoke to a child in an inappropriate manner, and did not report her actions to the director.

A caregiver told a six-year-old child they could "F....g Die" while they were in time-out.

A caregiver used the "F" word when speaking to a child who was in time out. The caregiver's inappropriate language was heard by other children and a staff member in the classroom.

Based on a DFPS investigation, it was determined that a caregiver turned away from the diaper changing table and did not have visual site or hand place on the child. The child jumped or fell from the table and received an injury.

One allergy plan was not signed by the parent.

During a DFPS Investigation at the operation on 3/7/2023, an infant was observed sleeping in swing.

Three classooms had napping children in the exit paths of classrooms. The violation was corrected at the time of inspection.

One infant was observed sleeping in a bounce seat.

Based on information obtained during the investigation, a caregiver failed to properly supervise a two-year-old child, which resulted in the child falling from a table and receiving a mouth injury.

One staff member did not have current training in SID's. Shaken Baby and Brain Development.

Three infants did not have current feeding schedules on file.

The operation failed to communicate with a parent in timely manner regarding a child who displayed symptoms of an arm injury.

A staff member stated she grabbed by the arm in order to redirect the child from running in the classroom.

A caregiver left seven two year old child unsupervised for approximately five to seven minutes. During this time, a child fell and received an injury to their mouth. Note: this was a room with half walls and the caregiver in the adjoining room was able to see the children and tended to the injured child?s need

Two staff members were observed out of their classrooms during the walk through inspection.

The operation failed to report an incident in which a child was able to exit their classroom and run out of the building.

It has been found the operation has failed to comply with the Laws and Rules for a Licensed Child Care Center.

A staff member failed to use good jugment when she did provide a proper account of an inident in which a child was able to run out of the building.

The operation did not provide a written incident report to the parent for a incidnet which placed a child at risk.

Based on the results of an investigation, it was determined that three children left the building and were unsupervised. The caregivers responsible for the group of children at the time were unaware that the children had exited the classroom multiple times through the emergency door and had exited the building.

Incident/Illness Report Form was not giving to parent after children left the operation unsupervised. Parents were not properly informed of an injury due to the Incident/Illness Report Form not written how the injury actually occurred in the classroom.

Based on the results of an investigation, it was determined that a caregiver spanked a child as punishment for leaving the building.

Operation failed to notify Child Care Regulation (Licensing) that children left the center unsupervised or that a caregiver was accused of hitting a child.

A caregiver failed to ensure that incoming staff into the classroom were fully aware that a parent had requested a change in the an infants feeding schedule.

Written feeding schedules for a chld no longer in care were not available for review at the time of inspection. This vioaltion was corrected at the tiem of inspection.

A staff member used inappropriate language in the presence of children. A caregiver was observed using a very harsh tone of voice when speaking to a child. A caregiver made an abusive comment to a child when they were moving around on the diaper changing table.

A caregiver used poor judgment when she sprayed pacifiers with a cleaning disinfectant. A caregiver used poor judgment when she trimmed a child's toenail after being instructed on two occasions not to trim the nail.

Two infants did not have current written feeding schedules. The violation was corrected at the time of inspection.
