The program director did not have current CPR/First Aid training on file at the time of inspection.

The director did not have current transporation safety training available for review at the time of inspection.

One caregiver did not have current CPR/first aid training. The training expired 01/2024. The site director's CPR/first aid certificate did not have a date to verify when it was completed.

At the time of inspection, there was no documentation that fire drills have been conducted since December 2022.

The operation did not have a current menu posted. The menu posted was from the month of December.

At the time of inspection, there was no documentation that the smoke detectors have been tested since December 2022.

At the time of inspection, there was no documentation that the fire extinguishers have been checked since December 2022.

The operation did not have a current gas leak test on file. The gas leak test was last conducted 11/2020.

At the time of inspection, there was no documentation that the carbon monoxide detector has been tested since December 2022.

A bottle of disinfectant spray was not labeled. (Note: This was corrected during the inspection when the bottle was labeled.)

Based on information obtained during the investigation, it was determined that two children were able to engage in inappropriate behavior due to a lack of supervision. The caregiver in charge was not aware of the children's ongoing activities and did not have visual and/or auditory awareness of the children.

The director did not have verification of current cpr/first aid training on file. The certificate was not available for review at the time of the inspection.

The director's personnel file did not include a notarized affidavit.

One child's record did not include the name, address, & telephone number for a physician or emergency care facility.
