During the course of the investigation, it was determined that a bathroom door was broken and fell on a child's hand which resulted in a child being injured.

During the course of an investigation, it was determined that a caregiver failed to use good judgement when a child received a critical injury that required medical attention and the caregiver decided to not call 911 which delayed the child receiving immediate medical attention. A caregiver also did not use good judgement when the parent was notified of the injury. The caregiver did not express the extent of the injury of the child to the parent.

During the course of an investigation, it was determined that the operation failed to call 911 when a child received a critical injury.

Three out of 10 caregivers lacked the required 24 hours of annual training. One caregiver lacked 7 hours, the second caregiver lacked 23 hours, and the third caregiver lacked 20.5 hours of training.

During the course of the investigation, it was determined that a caregiver was spanking children, pinching children, and hitting kids in the back of the head.

During the course of an investigation, it was determined that a caregiver was holding children by their feet for discipline practices and was throwing rocks at children.

The director lacked current training in child maltreatment.

The first aid kit was missing the required tweezers. This was corrected at inspection when the director added the tweezers to the first aid kit.

One employee record reviewed did not have the required 24 hours of annual training complete within their required annual training year.

Slides in the outdoor play area do not have the required six inches of loose fill being maintained under the slides.

Insect nests were observed under a play structure roof posing a safety hazard.
