Five records were reviewed. One child is missing medical authorization information on the enrollment form.

Bottles were not labeled in the infant room yet placed with the child's crib. Bottles need to be labeled or have an assigned chart. This was corrected at inspection; bottles were labeled.

A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Annual inspection has not been completed due to changes in staffing at Health Department (inspector called Health Department during inspection). Head of Health Department stated coming within the hour.

The infants hands are washed with wipes and not soap and water.

One child does not have the doctor signature for administering an Epi-Pen.

The infants do not have infant feeding instructions. There is a chart of how many ounces each age group (infant) should have, but not individual signed by the parent.

A tree stump is in the outdoor play area. This is a tripping hazard for the children and caregivers which can cause injury. The stump needs to be removed. The hole where the stump was will need to be filled to prevent further injury. (no children in care).

The gates to the outside play area need locks. This will prevent children from opening the gate during outdoor time and possibly running into the streets. Keys to the locks must be accessible to caregivers at all times during outdoor time. (no children in care)

The changing mat had a cloth cover. This was corrected at application inspection when the cover was taken off. (no children in care)

Cleaning supplies were under the sink without a child safety lock on the cabinet door. The cleaning supplies was accessible to children. This was corrected at inspection when the cleaning supplies were stored in a place inaccessible to children. (no children in care).
