Out of the children's files reviewed, 2 were lacking parent signature for emergency medical care.

Out of the children's records reviewed, 1 had incomplete information for the person authorized to pick up the children.

There were no staff file and training hours available for review.

Out of the children's files reviewed, 2 of the files reviewed had incomplete Dr's or hosptial information.

There were 3 files that did not have a health statement on file. The parent did not sign to confirm this information which would have provided time to obtain something in writing from the Dr which indicates that the child is healthy enough to participate in daycare activities.

There is one person listed as active on the people and is no longer employed at the operation.

The caregiver did not use good judgement by allowing 2 older children (ages 3-4) to sleep in high chairs as a way to keep them from crawling or walking on the children who are napping.

There was one caregiver supervising 2 rooms. One room was the infant room and the other room was where 2 children were napping (away from all others)

There was not enough lighting in any of the classrooms where the children were napping

There is a wood panel along the fence that is no longer attached and is causing a gap which could allow a small child to crawl through or an animal to get into the playground. There was also a lil tykes play structure that was crack near an opening. The crack could allow a child's finger or clothing to get stuck and cause an injury to the child. (NOTE:Pictures on file)

There is no current health inspection. The last one was completed on 7/3/23.

The permit holder is not ensuring that minimum standards are being properly followed by staff as children are being placed in dark rooms in high chairs to sleep.

Out of all children's records reviewed 2 had incomplete information for the person to contact in case of an emergency. (NOTE: Corrected during inspection as the parent was called for the missing information. )

One of the staff files was missing the required affidavit.

Out of the staff records reviewed, 2 had no verification of a TB test.

Two children's are missing a Health Care Statement on file.

A review of the Employees Who Left Employment in 2020 Survey data from January 25, 2021, revealed that the center had not reported the number of employees who left employment in 2020 by January 15, 2021, as required.
