One 10mo infant feeding instruction was observed need to be re-certify.

The infant feeding instruction for two infants ages 4mo and 9mo were not available for review during the inspection.

Caregivers in the infant classroom did not know how many children they were responsible for during the inspection. This was correct when the caregiver and the director was made aware and recount the children in the classroom.

Bottles in the 12-24mo classroom were observed unlabeled with the children name during the inspection. This was corrected when the caregivers was made aware and labeled the children names.

One 11 month old infant was observed sleeing with a blanket covered their head during the inspection. Caregiver was made aware, the blanket was removed from the infant's crib.

This standard was recited during the follow up inspeciton when a four months old infant was observed sleeping on the electric swing during the inspection. Caregiver was made aware, the infant was removed from the restricted device.

The operation was determined to not operating in compliance with Mininmum Standards when repeated citations were given which increased the risks to children in care.

One child in the 2yo classroom was observed had a diaper changed and did not washed their hands. Caregiver was made aware and washed the child hands while on site.

A four months old infant was observed sleeping on the electric swing during the inspection. Caregiver was made aware, the infant was removed from the restricted device.

Children in the 4yo classroom were observed watching TV while napping and resting.

Caregiver in the Pre-K 4yo classroom did not know the age of the children that they're responsible for during the inspection walkthough

The Fire Extinguisher for the Tranportation bus was observed out of service and will need to be recharged

Several classrooms were observed not having adequate lighting during naptime

Two infants were observed napping in cribs that did not have their name labeled during the inspection This was corrected during the inspection.

The wall in the temporary classroom for the Infants was observed having chipped paint during the inspection walk through

Two infants were observed sleeping with the pacifier attachments that can potentially be a choking hazard. This was corrected during the inspection.

CCR was not notified when the operation adding more indoor space for the children in care. This was corrected when the director was made aware and sending a written request to CCR.

The use zone for a rotating piece of equipment is overlapping the use zone of a slide.

The operational policy is lacking a physical activity policy.

The loose fill surfacing on the PreK playground is less than two inches in the use zone of a slide, rocking and rotating equipment.
