The tiles in the kitchen have started coming loose due to a plumbing break.

3 of the records reviewed did not have a signed medical authorization. Date of enrollment was missing on 3 records.

A child's record emergency medicall section was incomplete. There was no dr. name, address, or phone number; and the hospital information was incomplete.

The outdoor space was observed to have parts of the chain link fence detached to the frame causing entrapment hazards for the children. A portion of the frame on the chain link fence is bent and protruding into the play area. The hand washing sink in the outdoor area was clogged and broken. The water fountain in the outdoor play area is non working and has cords hanging form the unit. The outside electrical outlet was not covered properly. A window facing the outdoor play area was broken and cracked. A small play structure requiring surfacing was outside of the surfacing zone. Missing swings were observed exposing loose chain. Broken swings with heavy metal latches were observed laying on a ledge in the outdoor play area, in reach of children.

It was found that records of children were not keep on file for the required 3 months after withdrawal.
