The operation was aware of a child inappropriately touching other children in the past, and due to lack of supervision it resulted in another incident of inappropriately touching.

The operation did not report to licensing an incident involving inappropriate touching between two children which placed the children at risk. The incident happened on the 19th of December, and the operation reported the incident to licensing on the 22nd of December

It was determined that a caregiver used prohibited punishment by pushing a child during discipline.

The operation failed to notify CCR of a communicable disease within the 48-hour required time frame.

The infant C classroom was over ratio by 4 children. There were 9 children with one caregiver. The specified age group was 12 17 months, which only allows for 5 children. The preschool classroom was over ratio by 3 children. There were 14 children with one caregiver. The specified age group was 2 years, which only allows for 11 children. Note: The children were moved around in order to come into compliance with ratios.

Based on the information obtained during the investigation, it was determined that a caregiver did not follow written feeding instructions that were provided by the parent. Cereal was added to the child's bottle without the parent's knowledge or permission.

One caregiver only had 11 of the required 24 clock hours of training for the previous training year available for review at the time of the inspection. (Note: The caregiver will need to make-up the training hours. The make-up hours will not count towards the current training year)

One caregiver did not have training in the following topics: (1) emergency preparedness (2) Preventing and controlling the spread of communicable diseases, (3) Administering medication (4) Preventing and responding to emergencies due to food or an allergic reaction (5) Understanding building and physical premises safety (6)Handling, storing, and disposing of hazardous materials

Three classrooms (preschool, preschool discovery, and pre-k) did not have an emergency evacuation diagram posted.

Three infants did not have current written feeding instructions on file.

A monthly playground checklist has not been completed since the month of April. (Note: Inspections are being conducted but have not been documented)

Three of the ten children's records evaluated did not have a health statemetn on file.

Based on information obtained during a DFPS investigation, it was determined that caregivers were unaware of injuries a child sustained while in care.

The operation did not have a current certification form showing recalls have been reviewed. (Note: The director corrected this during the inspection when the form was completed & signed. The form is now available for review.)

There was no emergency evacuation and relocation diagram posted in the toddler classroom. (Note: The diagram was posted during the inspection.)

The operation did not have a current fire inspection on file. The last fire inspection was completed 2/2021.

The following hazards were observed: 1. There was a bottle of disinfectant spray sitting on the toilet in the toddler classroom accessible to children. (Note: The caregiver moved the chemical and put it in a cabinet that was inaccessible to the children.) 2. There were pieces of a broken toilet tank cover on the floor in one of the preschool restrooms. (Note: This was removed during the inspection.)

It was determined that on more than one occasion, infant children have remained in a restrictive device for more than the 15 minutes allowed by the Standards.

Incident reports were not available for review at the time of the inspection.

There was no gas pipe test documentation available at the time of inspection.

One child's file lacked immunization documentation.

One staff persons personnel file was not available at the time of inspection.

Three children's files lacked complete admission documentation.

It was determined that a caregiver hit a child on the back of the head, with an open hand.
