Two bathrooms in two different classrooms were observed to have the based board peeling and detached from the wall.

Cribs that are being used by infants was observed to not have snug fitted sheets.

5 out 10 children's records evaluated did not have a health statement in their file.

3 out of 10 staff records evaluated did not have documentation showing education credentials.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

5 out of 5 staff files reviewed showed caregivers and the director with online CPR certification that does not meet minimum standard requirements.

2 caregivers hired within the last year did not have the pre-employment affidavit available for review in their file.

10 out of 10 children's files reviewed did not have proof that parents had received information related to parent's rights contained within the operational policies.

2 out of 5 staff files reviewed did not show that they had completed the required health and safety topics in their most recently completed training year.

A caregiver hired 9/9/24 with no previous childcare experience did not have proof that 8 hours of preservice had been completed prior to being counted in ratio.

One child did not have a copy of their must current immunizations on file and was missing 3 required immunizations.

A caregiver hired 9/8/24 did not have proof that orientation had been completed within 7 days.

2 out of 10 records reviewed for children were missing immunization records.

It was observed one infant was sleeping in the crib with mattress sheet that was not not snugly fitted in Mobile Infant classroom. NOTE: This was corrected, the caregiver fixed the fitted sheet during the inspection.

The training record for one employee in Red Room classroom was not available for reviewed. The caregiver was hired since August 2021.

An 6 month old infant was observed sleeping in the bouncer during the inspection. NOTE: The infant was being removed from the bouncer during the inspection.

Two Infant feeding Instruction Forms for two Infants ages 11mo and 13mo were observed not being updated. The last forms was observed completed by parents since September 2021.

The transportation training for one caregiver was not available for review.

Several water bottles were observed not being labeled with children name in the Non-Mobile Infants classroom; Red Room Primary Classroom. NOTE: This was corrected as the bottles were labeled with the children names during the inspection.

Based on the information obtained, there is sufficient evidence to suggest that a report to CCR was not made within the required timeframe. Note: This was corrected when CCR was made aware of the situation that required reporting.
