At time of inpsection, one classroom television was not mounted or anchored. Noted: Television was removed from daycare by Director.

Follow up 12/11/24: There still is not 6 inches of loose fill under or around the use zones of the climbing equipment on the playground. There is only a max of 3 inches during the follow up.

Follow up 12/11/24: The use zone for the little tykes climbing structure with slides still does not extend six feet from the exit of the slide or the other side of the structure where the child climbs up on the structure.

An 11 month old infant was asleep using a boppie pillow. A blanket was also hanging over the edge of the crib causing it to fall into the crib.

Follow up 11/22/24: During the follow up the two little tykes climbing structures with slides have their use zones over lapping each other and one of the slides is too close the the wall.

Follow up 11/22/24: One of the tables is still broken. The rails on the little tyke structure have been bolted down and are no longer a hazards.

Two broken tables and a broken car were observed on the playground. The little tykes play structure is missing bolts on the side rails.

The director's file does not have the 30 hours of annual training for the training year 2023 to 2024.

The fence for the playground has a gap in the exit gates that is approxamately 5 to 6 inches and a small child can fit through it. The fence does not block off the stairs that go to the next door neighbor's appartment.

A review of the Employees Who Left in 2022 data reveal that the center had not reported the number of employees who left employment in 2022 by January 15,2023, as required.

There was one infant in care from ages 7 months and the television was on playing Cocomelon videos. The caregiver corrected this at inspection when she turned the television off.

While 10 children were in care the only caregiver was in the kitchen area while the children were in the classroom unattended. The caregiver left the children unattended multiple times.

The playground is still cluttered with toys and items making it a tripping hazard. More photos were taken.

Caregivers with hire dates 09/14/2011 and 09/11/2017 do not have the required annual training: -topics for the last annual training year.
