Three out of five classrooms had a screen with a moving image during naptime. This was corrected at inspection when the screens were turned off.

Based on the preponderance of evidence gathered during this investigation, a caregiver exercised poor judgement when they showed a video of an upset child in care to an individual not associated with the daycare on their personal device.

School-age children did not wash their hands prior to sitting down for snack. This was corrected at inspection when it was addressed with the caregiver and the children washed their hands.

A caregiver did not wash their hands after changing a child's diaper. This was corrected at inspection when it was addressed with the caregiver and the caregiver washed their hands.

The diaper changing area was not sanitized in between diaper changes. This was corrected at inspection when it was addressed with the caregiver and the diaper changing area was then sanitized.

There was a floor tile that was broken and lifting up in an infant room. An infant had a piece of tile in their mouth.

4 outlets in the pre-k room were not covered. This was corrected at inspection when outlet safety covers were placed over the outlets.

A child's hands were not washed after having their diaper changed. This was corrected at inspection when it was addressed with the caregiver and the child's hands were washed.

A 4 year old child was seperated from the group for time out for approximately 30 minutes.

A dog was brought into the center and they allowed the children around this dog without notifying the parents in writing.

A 4 year old child was asked during time out to stand still on a tape on the floor for about 4 minutes to approximately 30 minutes.

The director allowed a child to be put in time out in the kitchen, knowing it violated minimum standards.

A caregiver had a 4 year old child in the kitchen while they were cooking during time out. The kitchen contains chemicals, sharp objects and other items that must be kept out of reach of children.

The two year old caregiver was observed yelling at the children.

Two out of five infants had a bottle proped with a blanket.

One out of five infants was observed falling asleep while drinking a bottle in the crib.

One infant was observed sleeping with a pacifier with a stuffed animal attached to it. The caregiver corrected the deficiency by removing the pacifier with the stuffed animal.

The two year old caregiver was observed watching a video on a laptop while using headphones during naptime.

One 8 month old was observed to be sleeping in a restrictive device.

Two infants were observed to be swaddled while sleeping without the sleep exemption form.

Multiple infants were observed to be sleeping in the cribs with blankets covering them.

One infant was sleeping in a crib with yarn tied around the child's neck holding a pacifier.

A caregiver was observed wiping a child's nose and not washing her hands afterwards.

The play scape lacked four inches of the required six inches of loose fill.

Lighting was inadequate in the three year old classroom during naptime.
