Based on the information obtained during the DFPS investigation, sufficient evidence was available to support the provider demonstrated their inability to comply with 742.307(2)(a-e) by not having documentation available for review for each child they care for: (a) child's name and age; (b) the parent's home address and telephone number; (c) the names of other persons the child may be released to; (d) a list of each food the child is allergic to, possible symptoms if the child is exposed to the food, and the steps to take if the child has an allergic reaction; and (e) authorization to give the child medication, if needed.

Based on the observations made during the DFPS investigation, dried up food was observed on the door leading to the children's play area.

Two rooms, both with their doors partially opened and accessible to children, was filled with clutter from the floor to the ceiling.
