Out of six caregiver files reviewed, one caregiver did not have a notarized affidavit.

The operation did not notify Child Care Regulation and submit required documentation to list someone as a controlling person within the required time frame.

The operation did not have a current fire inspection available.

The operation did not have a current sanitation inspection.

1 out of 6 children records reviewed were missing complete vaccination records.

There is no documentation that the fire marshal approved the corrections made from the 5/19/22 fire inspection.

During the inspection, One infant was observed in a crib outside of the classroom in the hallway. One caregiver was seen standing in the doorway supervising one child inside of the classroom and one child asleep in a crib in the hallway, steps away from the classroom. Two caregivers were seen outside of the classroom during naptime leaving the children in the rooms unsupervised.

There were bottles and breast milk stored without the proper labeling as according to this rule. There were 2 of 4 containers of breastmilk with no labeling. There were bottles being used with only the child's first name, but not the last name initial as required. *This was corrected at inspection when all bottles and breast milk were properly labeled.

A caregiver was observed putting an infant in the crib with a full bottle. The caregiver walked away leaving the infant in the crib with the bottle. *This was corrected at inspection when guidance was provided and the infant was removed from the crib and held to be fed properly.

A subject no longer associated with the operation as of March of 2020 was still listed as active on the Background Check/People List.

The operation did not have a diaper changing station in the classroom with infant and toddlers. There was no surface area present to indicate the proper diapering is conducted as according to the rule. *This was corrected at inspection when the operation added a non-absorbent mat that is designated for diaper changing only. There will be 2 caregiver staff present during diaper changing to ensure proper supervision.

The operation did not have the most recent Form 2936 Licensing Inspection Report for 3/16/2020 posted as required. *This was corrected at inspection when the operation posted the final inspection report from 3/16/2021.
