Four out of four employee records evaluated were lacking annual training in the topic of emergency preparedness, medication administration, food allergies, etc.

The children's products annual certification was not completed. This was corrected at inspection when the certification was completed.

The television in the after-school room was not anchored.

The playground loose fill was observed to have dog feces.

Two out of ten children's immunizations reviewed needed to be updated.

The small playstructure with a slide did not have a 6 foot use zone. There was another structure within 5 feet of each other and they both required a 6 ft use zone. The climbing structure in the after school playground had another play item within the 6 foot use zone.

Nap mats were not labeled with a child's name or a number. CI: This was corrected at inspection when a number system was created.

It was confirmed a bucket was used by children during school bus rides. CI: During 09/01/22 inspection no bucket was observed in the school bus.

Five out of ten children's records lacked the special needs section.

The fire extingusher lacked a current servicing.

Three out of three drivers lacked a current transportation training.

Ten out of ten caregivers lacked a current SIDS training certificate.

The front and two middle playgrounds lacked the proper depth of loose fill under the structures with slides.
