San Angelo YMCA - Day

During walkthrough when caregiver was asked about the number of children present, caregiver paused and was observed counting the children before answered.

Caregiver stood out of the classroom when sending children to the bathroom. Caregiver was observed supervising children in the classroom and in the bathroom. Even when standing out of the classroom, caregiver was not able to see what was happening inside of the bathroom. Another caregiver was observed standing by the gym door entrance supervising children that were using the bathroom and children at the playground. Children at the playground were with a high school student.

A bottle filled with disenfectant was on a table, making it accessible to children in care. This was corrected when staff moved the bottle higher up making it inaccessible.

Child was left unsupervised in the cafeteria's bathroom, while caregiver in charge walked with the rest of the group to the classroom.

Caregivers were witnessed yelling at children.

Child had an incident causing a bruise on his cheek. Caregivers were unaware and parent was not notified.

A movie was offered as one of the activities for children in group yellow and orange. Movie was not related to the theme or activity plan for the day.

A caregiver did not roll call her children when leaving the classroom to transition to outdoor time.

Children arrived from outdoor time and were observed sitting at the snack tables without washing their hands. Children's hands were on the tables. Tables were not sanitize before snack time.

Last Lockdown drill recorded was completed on 9/21/20. During inspection, site director conducted a lockdown drill.

YMCA designee was not able to provide proof of 2 hours transportation training for YMCA bus driver who transport children from Blackshear Head Start to YMCA Day site.

Site director arrived from outdoor time and was not observed washing hands. Site director was observed serving children's snack.
