During the investigation, it was found infant daily reports were not being provided to the parents.

The use zone for a stationary structure was not 6 feet from the piece of equipment to the fence at the front end of the vehicle.

A group of school age children were not being adaquetly supervised when the caregiver was in the other classroom.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One caregiver was missing the affidavit.

A child was sitting in a high chair and was not strapped in. The child was too big for the straps to fit around the child.

The walls in the operation have both large and small areas where the paint is chipped in multiple places. The walls need to be cleaned, and the window tint in the front of the toddler room is peeling off the window.

I observed a changing table near the restroom that had a rip in it.

The following items in the refrigerator were expired. Gallon of milk 11/5/24 Chocolate Syrup 2/1/24 Mustard 5/9/24, Salad Dressing 1/24/2024.

I observed situations in which children could have been seriously hurt or injured while the caregiver was standing there.

The fire extinguisher was observed in the red. A photo was taken.

There were two potty chairs observed to not be sanitized after use. There was urine left in the potty chairs.

Based on the information obtained during the DFPS investigation, a caregiver was seen picking infants up by their arms from the changing table and swinging the infants to their back.

Follow Up: The youngest child in the classroom is 14 months old and the oldest child is 4 years old, that is more than an 18 month gap.

In the infant classroom, the diaper changing mat was torn.

One caregiver was supervising 22 children and the SAG was 2.

Children under 2 years old was observed watching television during the inspection.

There was no fire drill conducted for the month of March 2024.

Six children files out of six reviewed is missing the child s ability to swim without assistance documentation required by the new admission forms.

The monthly drills have not been documented for December. This was corrected at inspection when the director the drills.

The school age teacher was answering the daycare phone and assisting with parent pick up.

A 5 month old infant was combined with a group of children that had children up to five years old. This was corrected at inspection when the infant and younger toddlers were moved into a seperate classroom.

4 children not eating were seated in a high chair for more than 15 minutes with no activity. This was corrected at inspection when the children were removed and activty provided.

Two children were missing health statements.

A 16 month child was in a classroom with a 4 year old child. This was corrected at inspection when the child was moved.

The school age classroom was observed to be out of control.

A nap mat for one child was not labeled. This was corrected when the childs name and mat number was labeled.

The infant caregiver did not wash the infant's hands after diapering. This was corrected at inspection when caregiver washed the child's hands.

The director's child did not have an updated infant feeding sheet. This was corrected at inspection when the infant's parent (the director) signed the form.

During follow up inspection hand washing sink in infant room was not on. Based on additional information hand sanitizer has been used as a form to wash hands and the kitchen sink is being used.

During investigation inspection, 1 out of the 3 classrooms was out of ratio. It was one caregiver to 11 infants. Youngest child was 13 months and the oldest is 1 years old. SAG is 1 years old.

During investigation inspection, trash can with no lid was overflowing. Potatoe Tots where inside the trash can. Noted: Pictures were taken.

During investigation inspection, Health Inspector sent a letter on May 17, 2019 stating operation does not need a permit due to food being catered. Operation has not been following letter and has been preparing frozen food. Noted: Pictures were taken.

Based on external documentation yelling as a way to correct behavior has been used on children in care.

During follow up inspection, children are still running out of classrooms into other classrooms.

During investigation inspection, carbon monoxide, smoke detector and fire extinguishers were not checked for the month of April 2021.

Children with birth dates 1/27/19, 3/27/19, 8/15/15 and 2/1/18 were missing address for emergency contact person.

During investigation inspection, two staff members who transport children did not have proof of transportation training.
