The operation failed to submit the number of employees that left the operation in 2024 via provider portal.

I did not observe an address for the emergency contact person documented in 1 out of 10 children's records. NOTE: This was corrected during the inspection.

I observed classrooms of children sitting down to eat lunch without first washing their hands.

I did not observe documentation of pre-service training in one out of five personnel records evaluated.

I observed documentation of 22 of the required 24 hours of annual training for the 2021-22 training year.

During the routine monitoring inspection conducted on 8-23-22, I received information concerning an accidental injury that occurred while a child was in care. The injury required medical treatment, but the incident was not reported to HHSC - Child Care Regulation. NOTE: This was corrected during the inspection.

I did not observe an activity schedule posted in the Toddler I classroom.

I observed a caregiver changing children's diapers without washing her or the children's hands afterwards.

I observed infant feeding instructions for two infants that were not dated.

I did not observe thermometers in the first aid kits on the busses used for transportation. NOTE: This was corrected during the inspection.

I did not observe current transportation training for the director.

I did not observe the meeting place for severe weather drills, inside the building, documented on the emergency evacuation diagram.
