There was 1 infant feeding instructions that had not been updated since 7/02/2025.

A caregiver stated she had 8 children and it was determined she had 7 children in care.

During current inspection, a classroom was outdoors and a child was not visible to the caregiver in the classroom.

During current inspection, I observed a balance beam without proper loose fill surrounding it. The Director removed the beam during the inspection.

It was determined children under 5 years old were allowed to play in the front of the operation, that is not fenced.

Based on the evidence from the DFPS investigation, it was determined a caregiver neglected 3 children in care.

A caregiver did not intervene during an incident when children were acting inappropriately each other.

Based on the evidence from the DFPS Investigation it was determined that 2 caregivers have been observed utilizing their cell phones while caring for children.

It was determined the operation was not ensuring the supervision of children and caregivers were not able to intervene to reduce the injuries to children in care.

Three personnel files reviewed did not have a notarized affidavit on file for review.

A caregiver did not have documentation of CPR/First Aid completed on file.

A caregiver observed counted in ratio did not have documentation of pre-service on file for review.

One caregiver had only completed 6 out of the required 24 training hours completed for the year.

There were 2 children's file that did not have a health statement on file.

One caregiver's file did not have documentation that orientation had been completed.

There were 2 children's files that did not have immunization records on file.

An infant in care had not had their infant feeding instructions updated since November 2024.

A book shelf in the infant classroom was observed to not have been anchored to the wall.

Two caregivers did not have proof of educational requirements on file for review.

A changing table was observed to have crumbs and dirt under the changing pad.

It was determined the incident occurred on 12/10/24 and the incident was not reported to Child Care Regulation until 1/03/25.

The first aid kit was missing antiseptic wipes and tweezers.

Based on evidence from the DFPS investigation, it was determined a caregiver yelled at children in care while disciplining them.

Based on the DFPS Investigation, it was determined that a child in care was abused.

Based on evidence from a DFPS investigation, it was determined a caregiver grabbed a child by their shirt.

Six out of siz files reviewed did not have a health statement for review.

One child's file did not have the immunization record on file for review.

There were 2 infants (12 months) observed in a classroom that was not regulated for infants.

An 11 month old infant was observed sleeping on a mat.

There were 2 caregivers that did not know the number of children they were responsible for. One caregiver stated she has 9 children and it was determined she had 10. Another caregiver stated she had 8 children and it was determined she had 9 children in care.

A caregiver was observed vacuuming while the children were sleeping and the children were not being properly supervised.

A caregiver was unaware of the number of children in their group.

Director allowed children to go to a building that was not part of the operation's floor plan.

A caregiver was observed taking children inside and the caregiver did not inform the co-teacher the number of children she was taking with her.

A caregiver was observed asleep next to a child the caregiver was putting to sleep during naptime. The caregiver woke up when the door was opened.
