The fire extinguisher in van #2 was not secured in the passenger compartment. The fire extinguisher was located in the door pocket of the driver's side of the van.

One caregiver who was supervising a group of children on a field trip did not have current documentation for First Aid/CPR. Caregiver has current training, but documentation was not available.

The use zone for one of the slides was less than 6 feet. The barrier for the loose fill was in the area. This slide must be made inaccesssible to the children until this repair can be made.

One employee is missing 22 hours for year 2/20/2020-2021. One employee is missing 24 hours for year 9/5/2019-2020. One employee is missing 21 hours for year 1/8/2020-2021. One employee is missing 17 hours for year 6/26/2019-2020.
