It was observed that a slide that is 27 inches tall was lacking loose fill around the slide.

It was observed on the playground there were three broken toys on two playgrounds, there were cleaning supplies in the reach of children in two classrooms, there was a bag of garbage that had been placed in a children's restroom on the floor, and that there was trash on the floor of another classroom. The cleaning supplies was moved to an uppershelf in both classrooms.

It was observed in the infant room that five of then cribs had loose fitted sheets on the mattresses. This was corrected when the director removed the crib sheets from the mattresses.

During the walkthrough it was observed that a television in a classroom that is used on occassion was not anchored or secured which could lead to it falling over on a child.

During walkthrough it was observed that there was cleaning supplies and household paint accessible to children. It was also observed that play equipment on toddler playground had two broken seats. Corrected at inspection when director/staff removed cleaning supplies and paint out of reach of children. Director placed duck tape on broken seats of equipment.

This standard was reviewed as part of an investigation and found non-compliant. It was discovered that several staff while outside on playground had positioned themselves in one location away from children, behind equipment or had their backs to the children.

This standard was reviewed as part of an investigation and found non-compliant. It was discovered that a staff had used harsh voice when telling a child to sit down in a chair.

This standard was reviewed as part of an investigation and found non-compliant. It was discovered that a child had picked up a label for labeling bottles and cubbies and placed it in their mouth.

During the walk through of playground it was observed that there were broken toys that may cause an injury. This was corrected at inspection when the director removed from the playground.

This standard was investigated as part of an DFPS investigation and found to be in non-compliance. It was discovered that a staff had moved a child's foot with their foot and when the child kicked the staff, they grabbed the child and pulled them to the table by their arm.

This standard was reviewed as part of an investigation and found non-compliant. During the investigation it was observed in video provided that staff did not intervene when two children were fighting over a toy that led to a child biting another child.

It was observed in a toddler room that a caregiver did not wash their hands after changing a child's pull up/diaper.

It was observed during walkthrough that a uncovered trash can present in a Pre-K's restroom had soiled pull up accessible to children.

It was observed that a child did not wash their hands after having their pull up/diaper changed in the toddler classroom.

During a review of operational policies it was lacking policies on the following: Benefits of physical activity and outdoor play, duration of physical activities both indoor and outdoor, setting in which physical activity will take place, recommendation of clothing and footwear, and a plan to ensure physical activites occur when weather conditions prevents outdoor play.
