There was animal poop visible in one of the outdoor playgrounds on the floor and in two wooden boxes filled with dirt that are accessible to the children. Note: This was corrected by picking up the poop and disposing of the dirt that was in the wooden boxes.

An infant that is not eating table foods did not have written feeding instructions.

The menu that was posted was not up to date, the date on the menu was April 4/24/2023. NOTE: This was corrected at inspection when They printed and updated the menu.

(27) Procedures for conducting health checks, if applicable (28) Information on vaccine-preventable diseases for employees (29) If your operation administers unassigned epinephrine auto injectors to use in case of an emergency Anaphylaxis reaction. (30) Procedures for supporting inclusive services to children with special care needs.

A review of the Employees Who Left Employment in 2022 data from January 26, 2023, revealed that the center had not reported the number of employees who left employment in 2022, as required.

There were several sets of shorts and undies in the restroom hanging up to dry.

The trash can in the toddler room did not have a cover and allowed all soiled diapers to be expossed. (NOTE: Corrected during inspection by throwing out all diapers)

Electronic fire alarm was last inspected on 9/30/2020.

A 6 month old infant was observed with a monkey plush toy inside crib and a small hand sanitizer container between the sheet and mattress. Note: This was corrected during inspection as both objects were removed and crib was left bare with proper fitted sheet.

One out of four records reviewed was missing the following trainings: - Handling, storing, and disposing of hazardous materials. - Understanding building and physical premises safety.

Children were observed sleeping on cots listed under other children's names.

Fire extinguishers were last serviced on 9/2020. Note: This was corrected during inspection as fire extinguishers were serviced during the inspection.
