During this investigation, this standard was cited due to staff using poor judgement when using a small elastic hair band to hold the socks up on an infant. This action caused the infant to sustain skin depression from the tightness around their ankles. Pictures received for this investigation revealed the use of a small elastic hair band placed around the top area of the sock by the ankles causing the elastic band imprint.

The child safety alarm on the bus is not working at this time.

The reporting of the number of employees that left in 2024 for operations was due on 1/15/2025.

Fire drills were not conducted monthly and were not available at the time of inpection.

The caregiver stepped out of the classroom to call the director, leaving the children unattended.

The fire inspection was expired at the time of review 9/11/2023.

2 out of the 5 children immunization records were not current at the time of inspection.

Caregiver did not have First Aid training within 90 days of employment and currently does not have one.

The health inspection was expired at the time of inspection 8/22/2023.

Caregiver did not have CPR training within 90 days of employment and currently does not have one.

Four out of four children's files reviewed were missing required information including: Emergency contact addresses; Dates of Admission; and Healthcare Provider Statements.

One infant was observed in a highchair without using the required safety straps. This was corrected at inspection when the child was removed from the chair.

One infant feeding schedule observed was not updated and signed by parents within the last thirty days as required.

Two out of Four employee records reviewed were missing the required documentation of educational qualifications.
