During the inspection it was determined that documentation for the operations emergency drills has not been completed for the current year.

During the insepction screen time was used to replace activities from the operations lesson plan.

During the inspection the annual certification for CPSC was not completed as stated by the provider.

During the inspection only 7 hours of annual training for the Primary Caregiver/Director were available for review.

A trash can with used diapers was observed in the classroom and in reach of children. This was corrected at inspection when it was moved out of reach of the children.

Both caregivers did not have an affidavit for review during the inspection.

During the inspection 5 children records were reviewed, multiple record were observed missing information for the Emergency Medical Authorization. 4 children were missing information regarding the release of children. 2 children were missing a Health Statement.

During the inspection a container of cleaning/disinfecting wipes were on the floor in reach of children.

The primary caregiver did not have current CPR/First aid certification. The certification expired in January 2021.

The training materials were not provided within the 5 day time frame as requested for the previous year.

During inspection the operational policies were reviewed. There was not a policy in place for safe sleep or activities.

During inspection 4 children records were reviewed. The records were not maintained and the following was missing. 3 missing the health care information, 2 missing the address for emergency care, 1 was not signed for special care statement, 1 had not signed off on operational policies, 3 did not have a health statement from the physician and 2 did not have current immunizations. This was discussed with the primary caregiver.
