Based on information obtained from observations from video footage, a caregiver was observed aggressively grabbing a child by both arms, picking the child up, and sitting the child forcefully down on the floor. The caregiver was observed to grab the child again to separate them from the group and then forcefully sit the child on the floor against a wall adjacent to the group.

Four infants do have feeding instructions signed and dated by the parent or health care professional.

A rotating play structure on which a child sits does not have a use zone of a least six feet.

An initial background check was not completed for a caregiver who was caring for children during the inspection. This was corrected during the inspection as the caregiver went home. A background check was submitted for the caregiver during the inspection.

This standard was evaluated during the investigation and was found to be deficient. The caregiver used a prohibited punishment on a child in care.
