A school-age teacher was present by themselves with twenty-seven children. The youngest child was six and the oldest child was twelve. This was corrected during the inspection when the director moved a child to another classroom.

There were multiple mushrooms growing out of the ground outside on the playground. This was corrected during the inspection when i observed the career remove the mushrooms.

Two caregivers in charge of 24 children 21mo-5years old was not aware of the number of children in their care. When I asked one caregiver they stated there were 23 children and then the other caregiver corrected her and stated no there's 25 children. However, I counted 24 children. The caregiver was asked again to verify. The caregiver recounted and stated no there is 25 children. However I recounted again and there were 24 children. The caregiver then recounted again and she counted 24 children and stated one of the kids had went home.

School age children were observed playing video games on a computer. The games were not educational or apart of the planned activities.

The cribs were all next to each other with no room to walk in between. This was corrected when the director, Rose Gainey, rearrranged the furniture while I was present.

The caregiver in the toddler class stated she had 24 children in care, but there were 25 children present. This was corrected during the inspection when the caregiver recounted the children.

The operation could not provide documentation of liability insurance coverage for review by licensing. The previous documents stated insurance coverage expired 04/28/2023.

Based on the DFPS investigation, there is sufficient evidence to suggest that supervision was not maintained when a child was left on a vehicle and then wandered away from the operation.

Based on the DFPS investigation, there is sufficient evidence to suggest that a caregiver did not account for all children causing a child to be left on the vehicle.

Based on the DFPS investigation, there is sufficient evidence to suggest that neglectful supervision occurred at the operation when a child was found by a bystander away from the operation.

Based on the DFPS investigation, there is sufficient evidence to suggest that the driver of the vehicle did not conduct a walk through or visual check of the inside of the vehicle after dropping the children off at school.

Notarized affidavit was missing on one out of four staff files checked. This was corrected during inspection when caregiver went and notarized the form.

Current employee list did not have two of the names of caregivers present during inspection. This was corrected during inspection.

Daily menu was not posted. This was corrected during inspection.
