Primrose School of The Woodlands at Creekside Park

Based on information gathered during inspection a child was left outside on the playground unsupervised.

The stairs on the Pre K playground have been repaired. In the PPK1 classroom there was peeling paint on the wall near the floor; accessible to the children. Note: A picture was taken.

On the Pre K playground there was material on the stairs to the play equipment that was torn and caused a tripping hazard.

The caregiver in the Kinder Classroom stated several times that there were 12 children in care but there were actually 11 children in the classroom.

Based on the information obtained during the investigation a child was left unsupervised in a classroom.

I evaluated 10 children's records. I observed the following: * One without documentation of contact information for the child's physician. * Two without documentation of health statement * Two without complete address for the emergency contact person

I did not observe current feeding instructions for two infants not yet ready for table food.

Interviews were conducted and transition procedures were evaluated. It was determined that a preschool age child was left on the playground without adult supervision when the class transitioned back inside to the classroom.

During a recent DFPS investigation, it was determined that the operation was neglectful in addressing injuries that a child sustained while in care.

During a recent DFPS investigation, it was determined that the parent of the injured child was not notified in a timely manner of the injury that the child sustained while in care at the operation.

I did not observe documentation of vision and hearing screening for a 4 yr. old child in care at the operation.

I observed a caregiver in an infant class change a child's diaper without washing the child's hands afterwards.
