A four-year-old child was left alone in a classroom for approximately 5 minutes.

A caregiver did not follow operation's procedure when transition from classroom to outside playground. As a result, a child was left unattended in the room for approximately 5 minutes. The same caregiver also did not notify management of the incident.

A caregiver was seen hitting a child's hand to reprimand them.

Child was not given his 4:30pm bottle.

Operation did not keep required record for three months.

A child was left unsupervised on the playground for a short amount of time.

One of two food allergy emergency plans was no signed by parents as required.

Pre-service training should be viewed as a support to the caregiver who has taken on the responsibility of caring for children for the first time. Research indicates without some basic understanding of children and how to work with them, the health, safety, and well-being of the children in care are at risk.

One caregiver, whose file I observed, has not taken orientation training as indicated by the personnel form.

6 of the 10 caregiver files that I reviewed were missing photo identification.

The employee list was not updated at the time of inspection. New employees were added to update the list and old employees were taken off.

I observed bleach water in a spray bottle on the counter of the hand washing sink in the pre-k classroom. The teacher put the chemicals in the upper cabinet, where it was not accessible to children.

3 of the 10 caregivers files I reviewed was missing or had expired CPR/First Aid training.

The vehicles used to tranpsort the children do not have safety alarms.

The staff who tansport and the director did not have current transportation training.
