A caregiver was seen by management hitting a child on the leg. The child stated that the caregiver hit her on her leg during my interview with the child. This was corrected at inspection. The caregiver has been terminated.

A child was left in the bathroom/classroom alone. This was corrected at inspection. The caregiver has been terminated.

The operation did not report a suspected diagnosis of Salmonella that was reported by a parent about their child.

Based on information obtained, it was determined that staff did not ensure proper supervision at the time of incident.

A child was left in a room alone without adult supervision. This was corrected at inspection when the director provided licensing with documentation showing that they have implemented a week long protocol requiring each class to contact management when they transition. Management examines each classes transitions throughout the day. Caregivers have been terminated.

The caregiver did not demonstrate good judgement when she put her hand inside of a child's pants to feel if he was soiled with urine.

A young child was left on the playground unattended for five minutes.

The first aid kit on the buses were missing antiseptic wipes, thermometer, and tweezers.

The restroom in the preschool classroom toilet seats are discolored.

The fire inspection expired on July 5, 2022.

The opertaion has not documented their emergency drills since November, 2021.

During inspection of the Pre K classroom three were 25 children present in the classroom with one caregiver. The specificate age group of the children was five years of age. The second caregiver needed to meet ratio requirements had left the classroom for a restroom break.

Training hours for nine staff members was available for review at the time of inspection.
