During inspection the loose fill on both playgrounds was found to be compacted with about 1 inch of loose fill on top. Both will need a full 6 inches added on top or dig out the compacted earth to be able to properly use the borders.

Six children's records were reviewed and the following corrections are needed: #1 and #6 need new form completed #2 needs address and phone number for emergency contact #4 needs parent to sign special needs box All needs a parent's right signature

A caregiver was observed pulling a child while disciplining, causing them to fall and receive an injury as well as popping a child on the hand. The caregiver was terminated immediately.

This standard is related to a DFPS Abuse/Neglect Investigation.

The health inspection is expired.

The fire inspection is expired.

Nine training records were reviewed and two staff do not have the required 24 hours of annual training for their invididual training year.

The back playground has a large bolt sticking out of the ramp that needs to be hammered down or removed. The front playground has a bolt on the climbing equipment that is loose and could cause a child to hang clothing or other item or to cut themselves. It needs to either be fixed or removed.

The sink in the preK classroom has a broken piece in the bottom with sharp edges. A toilet is not working and needs to be fixed.

Seven staff records were reviewed. 5 staff need additional required 6 hrs. 3 staff need current Abuse/Neglect Training 2 staff need SIDS/ShakenBaby/AHT/BD 1 staff needs transportation training All staff need Required Annual traiining that includes Emergency Prep, Medication, Bldg/Premises Safety, Comm Disease, Food Allergies,and Haz Mat. 5 staff need their full hours.
