At the time of the inspection 1 of the 10 children's records of immunization were not up to date with current requirements and an exemption affidavit was not on file at the operation.

At the time of inspection it was found that a part of the building had damage from a storm and construction was being conducted to repair the area.

At the time of the visit a tube of prescription ointment was present in a classroom. The prescription box was not present to say which child the tube belonged to or the dosage.

At the time of the inspection 1 of the 10 children's records reviewed did not have emergency medical authorization in their file.

At the time of the inspection the annual fire inspection had not been conducted within the last 12 months.

At the time of the inspection the director did not have up to date CPR/First Aid certification.

At the time of the inspection 1 of the 10 employee training records reveiwed did not have a CPR/First Aid certificate on file.

The operation allowed a caregiver to provide direct care to children as a second caregiver in the classroom prior to receiving a background check determination of, Eligible, Provisional Eligibility with Conditions, or Eligible with Conditions. Caregivers without background check were dismissed from the building before inspection was complete.
