This standard was reviewed as part of an investigation and found non-compliant. The incident report provided was lacking information required by licensing. The following was lacking on incident report: Director's name, operation number, if parent and licensing were notified (date, time, and by who), signature of director verifying they reviewed incident report and parent/guardian acknowledgement.

It was observed that four of the six bucket highchairs were lacking safety straps.

It was observed that a child with a food allergy was lacking an emeregency plan on file.

It was observed that the operation was lacking proper depth of loose fill on the playground.

A soiled pull-up was observed in a trash can without a lid. The soiled pull-up was also not in a sealed plastic bag.

An infant under 12 months of age was observed in a bouncer that was placed inside a crib sleeping. The child also had a pacifier with a pacifier attachment and a bib on while sleeping in the bouncer that was placed inside the crib. A second infant under 12 months of age was observed in a crib sleeping with a boppy pillow. This was corrected at inspection as the bouncer was removed from the crib and the infant was removed from the bouncer and placed in the crib. The bib and pacifier were removed. The boppy pillow was also removed from the crib during inspection.

An infant was observed sleeping in a bouncer that was placed inside a crib. This was corrected at inspection as the child was removed from the bouncer.

There was no staff member or caregiver in a classroom supervising infants and toddlers who were napping when I walked into the classroom at inspection. This was corrected at inspection as the caregiver returned to the classroom.

Two caregivers records lacked a notarized Affidavit for Applicants for Employment with a Licensed Operation or Registered Child-Care Home (Form 2985).

This standard was reviewed as part of a DFPS investigation and found non-compliant. An operation fails to timely report to CCR that a child was injured and required medical treatment by a health-care professional.

This standard was investigated as part of an DFPS investigation and found to be in non-compliance. The incident report was not complete with all required information and the parent/guardian had not signed.

This standard was reviewed as part of an investigation and found non-compliant. During the investigation it was determined that a caregiver hit the table to get a child to stop them from crying and hitting other children during lunch time.

This standard was reviewed as part of an investigation and found non-compliant. It was discovered during the investigation that staff were aware the carpet was fraying in the gym. Due to the frays of the carpet a child had placed a part of the fray around another child's neck causing a scratch to their neck.

It was observered in two seperate classrooms that the rubber baseboard was missing or broken exposing glue and sharp edges. (Photo was taken).

This standard was reviewed as part of an investigation and found non-compliant. During the investigation it was determined that a caregiver grabbed a child by their shirt/dress at the back of their neck area to remove from under a table.

During the investigation it was discovered that a staff had not treated a child with courtesy and respect when the staff pointed their finger at the child when they were trying to express themselves while in front of their own parent.

One of ten children's files was lacking a complete address for their physician.

The operation was lacking a current annual fire inspections. The last inspection was conducted on 8/12/21.

It was observed in the infant room that a baby walker was present. This was corrected at inspection when the director removed from classroom.

While reviewing training records staff were lacking training in required content. A list was left for director on trainings and staff.

During a review of children's files 1 of 12 children's files was lacking parent authorization and 2 of 12 were lacking a complete address for their emgency authorization for doctor or hospital.
