It was observed during walkthrough that a child was placed in front of an emergency exit during nap time. This was corrected when the child was woke up from naptime.

During the inspection it was observed that the loose-fill was not adequate of atleast six inches.

During the inspection the pool was green where the bottom of the pool was not visible.

It was discovered during the investigation that a caregiver had walked away from the changing table while a child was present on it to retrieve supplies. This was corrected immediately when the caregiver was terminated.

It was observed that several children were standing against the wall during active play outdoors. The children and staff verified that they were in time out for hitting each other while they were inside. This was corrected when the caregiver told the children they were able to leave timeout.

One of ten children's files reviewed was lacking a current immunization record and their immunization affidavit expired 2/22/20.
