A caregiver was observed in the classroom without an active background check. Note: This was corrected when the caregiver left the operation and their background was submitted.

During the inspection, one classroom's cots were not labled with children's names, or did not have the cots numbered accompanied with a numbered cot assignment map.

This standard was found deficient as part of the DFPS investigation.

A caregiver failed to account for all the children prior to doing diaper changing in the bathroom and shutting the door. Caregiver failed again to account for all the children prior to leaving the classroom, which resulted in leaving the child in the bathroom.

A child was left unattended for approximately 55 minutes.

Based on information gathered during the course of the investigation, it is determined a caregiver failed to supervise a three-year-old child and four-year-old child and were left unattended in the outdoor playground for approximately less than 3 minutes.

Based on information gathered during the course of the investigation, it was determined a combined classroom which included ages 3-5 years old was observed out of ratio with a total number of 24 children present with one caregiver.

Based on the DFPS investigation, it was determined that the director failed to ensure compliance with minimum standards.

Based on the DFPS investigation, it was determined that director observed video footage of the incident involving a caregiver grabbing a child and did not report to Child Care Regulation.

Based on the DFPS investigation, it was determined that director observed video footage of the incident involving caregiver picking up and grabbing a child incorrectly and did not report incident to parent.

Based on the DFPS investigation, a caregiver was observed, on two separate occasions, grabbing a child by one arm and picking them up off the floor.

Most recent fire inspection report dated 03/21/2024, corrections for the inspection report have not been completed for the sprinkler system that needs to be inspected.

The operation did not have the required documentation for the unitary surfacing materials in both playgrounds.

Based on interviews conducted, caregivers failed to supervise a two-year-old child who was left unattended in the outdoor play yard for approximately 5 minutes.

The staff did not conduct a name to face after entering the room from the playground to ensure all children were accounted for. This resulted in a child being left outside for approximately 7 minutes unsupervised. The child was retrieved from outdoors once the teachers realized a child was missing.

Two infants were missing an updated feeding instruction.

One out of five infant's bottles were not labeled with the first name last initial. This was corrected when the teacher labeled the bottles.

A two-year-old child was found in a dark empty room for an undisclosed amount of time while two teachers which one of the teachers was the child's family member and the other children were right outside the door in the big play area. It was said when the child was found sitting in the dark room that he was not in distress.

When a 2-year-old child was found in a dark empty room this situation was not reported to the director by the staff making her unaware of the situation for her to report it to Licensing.

The outdoor fence playground was not installed at the time of inspection.

The annual sanitation inspection was not available for review at the time of inspection.

The fire inspection report or letter was not available for review at the time of inspection.
