The gym has a bathroom door which has a hole in it. The outside playground area has an outlet that is coming out of the wall, which has a nail and wires exposed.

The centers operational policies were observed and missing the following; (28) Information on vaccine-preventable diseases for employees, and (30) Procedures for supporting inclusive services to children with special care needs.

The operation did not use good judgment by allowing three children with known food allergies to be present at the operation; without required medication to treat them if they were to come in contact with allergy.

The operation does not have documentation for required annual sanitation inspection.

The facility did not report to licensing an incident that placed a child at risk at the operation within the required timeframe of no later than two days.

The facility did not complete an incident report for a situation that placed a child at risk.

A caregiver did not use positive methods of discipline when they physically redirected a child. The caregiver forcefully picked up the child and got in the child's face.
