Based on the interviews with staff and children, it was found that caregivers are yelling at children as a form of discipline.

The after school group did not have a written activity plan. The Director is going to correct this by making an activity plan for the after schoolers.

There was no activity plan for the after school age group that included a screen time activity-the children were watching a movie. The Director correct this at the time of the inspection by turing off the movie.

There was a trash can in the after school room that contained food without a lid making the garbage accessible to children.

There were two infant feeding instructions that were not updated. The Director is going to correct this by getting the feeding instructions updated.

There were two cups in the toddler room that were not labeled with the child's name. The caregiver corrected this at the time of inspection by labeling the cups.

There are three pieces of climbing equipment on the playground that do not have sufficient amount of mulch. The Director has decided to remove the equipment and is going to not allow children to play on the equipment until all three pieces are removed.

There were two trash cans in two different bathrooms that did not have lids. The Director corrected this at the time of the inspection by replacing the trash can with trash cans with lids.

The fire inspection went out on 1-6-2022. The Director is going to correct this by getting the fire inspection done.
