Based on information gathered during the course of the investigation, a 15-month-old child fell from a 5 ft. 5 in. play structure platform.

The caregiver in the toddler classroom did not know the current ages of the children in care. CI: Management returned to the operation and used Pro care to provide licensing with the correct ages of the toddler classroom.
