12-17 month range group size was over ratio. 6 children observed with one caregiver. CORRECTED at inspection when a 12mo old child was moved to the infant room that had one child.

One child was left unsupervised sleeping in play yard as teacher gathered all other children from different room to move them to the infant area.

No routine observed while in opertation, Toddler room was in the afterschool room and activity plan was not being followed.

5 children were observed to be watching cartoons on TV not related to planned activities. NOTE: Caregiver turned TV off and began following activity plan.

An individual with a pending background check was observed inside a classroom having contact with children in care. Individual was left alone with children during two occasions during inspection. NOTE: Director had individual leave the operation until eligibility letter is received and background check is active.

Children's hands were not washed prior to eating their afternoon snacks. NOTE: This was corrected during inspection as caregiver was informed and she took children to wash their hands.

A caregiver reported she was supervising two groups of children in two different classrooms during nap time on this day. An infant slept on his crib in the infant classroom, and 9 other children napped in the classroom next door.

A caregiver was supervising two groups of children in two different classrooms during naptime. A 2 year old child was left by himself asleep in the infant classroom while caregiver was providing supervision of a group of children in a different classroom. NOTE: This was corrected during inspection. Caregiver moved all children to one classroom where supervision could be achieved.

There were 12 children under the care of 1 caregiver. One of the children was an 11 month old infant. Operation was deficient in child/caregiver ratio by 1 child. NOTE: This was corrected during inspection as a second caregiver arrived at the operation and group of children was divided into two.

A crib in use during inspection was not labeled with child's name.

CPR and 1st aid trainings were not available for review for one caregiver present during inspection.

Personnel and training records for one caregiver were not available for review during inspection.

TV was in use in the toddler classroom showing cartoons not related to planned activities. NOTE: This was corrected during inspection as TV was turned off.
