The director did not have current training in Transportation safety. The certificate available for review had a date of completion of 2/6/2024.

The first aid kit in the transportation vehicle was incomplete. The items missing were tweezers and waterproof, disposable gloves. Note: This was CORRECTED during inspection as the missing items were obtained and included in the first aid kit.

A caregiver did not wash an infant's hands after a diaper change. Note: This was addressed during inspection and caregiver proceeded to wash the infant's hands

The director was missing annual training on the following topics: Preventing and responding to emergencies due to food or an allergic reaction; and Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic.

1 of 6 children records reviewed was missing the statement of the child's health from a health-care professional.

There were no time sheets available for review for the director.

A caregiver who conducts transportation did not have current training in transportation safety.

Six pedals were missing from a children's merry-go-cyle in the outdoor playground.

There was no vehicle documentation to show the date of lease/purchase for the transportation vehicle requiring a child safety alarm.

The director and 3 caregivers did not have the following required annual training hours: -Child maltreatment -Emergency preparedness; -Administering medication, -Preventing and responding to emergencies due to food or an allergic reaction -Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic and -Handling, storing, and disposing of hazardous materials

The outdoor play area was observed with trash (plastic wrappers.) and big rocks. Corrected at Inspection: The outdoor play area was cleaned during the inspection.

One child's record review did not have a health statement on file.

Director did not have training records available for review.

The Infant Caregivers did not have the following annual training topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3) Understanding early childhood brain development. The person in charge of transportation did not have a transportation training in the file for review. A caregiver hired on 12/16/19 did not have Pre-Service training certificate in the file. Several training certificates did not have an assessment tool to show the knowledge acquired from the trainings.

The Spring Rider and Circle Cycle structure were observed with rusted area. There was an unused water heater outside in the outdoor play area. There was grass growing from the mulch. There was no mark for the mulch to indicate the depth of the surfacing material.

A 4 seat Spring Rider did not have the required use zone area.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

6 staff records were reviewed. The following required personnel records were not available for review: -Documentation showing educational qualifications for one caregiver. -A notarized Licensing Affidavit for one caregiver. -A statement signed and dated by the employee showing he has received a copy of the child-care center's policies for two caregivers. -A statement signed and dated by the employee verifying the date the employee attended training during orientation for two caregivers.

5 out of 6 caregiver records reviewed did not have the following trainings: -Emergency practices -Administering Medication -Food allergies -Understanding Building and Physical premises safety -Handling, storing Hazardous Materials -Abuse and Neglect training

Fire inspection expired 3/17/2021. New fire inspection has not been scheduled as of today.

4 infants were left unsupervised during nap time in the classroom as caregiver exit to use restroom. There was a floater caregiver at the operation but was not notified in order to offer coverage. NOTE: Caregiver returned from restroom break and supervision was resumed. Administrator talked to caregiver about supervision.

Written feeding instructions were not available for review for four infants not eating table food.

Children's products certification was not completed timely. Last completed form was on 7/2019.

Documentation for fire drill for the month of February 2021 was not available for review.

First aid training certificates were not available for review for 3 out of 3 employee records.

Two children were observed inside cribs assigned to other children by name. Both cribs were not labeled with children's names correctly. NOTE: This was corrected during inspection. Correct names were added to each crib.

Two diagrams posted did not have two exit paths marked on form. One diagram did not specify the inside relocation site.
