During review the home provider's CPR had expired.

During the inspection children were seen outside, while outside a bottle of flat tire fix that was marked keep out of reach was seen in the area.

Dog waste was seen in the outdoor play area.

This Standard was determined to be out of compliance when children's records could not be provided to the DFPS Investigator.

During the inspection the bathroom was observed, under the unlocked cabinet a bottle of dish soap and hand soap were seen within reach of children. Both bottles stated to keep out of reach of children.

During the inspection the outdoor play area was observed. During the observation a flee treatment, mouse pesticide foam and a can of gasoline were seen on the play area.

The outside play are had multiple piles of dog feces in it.

Adequate lighting was not observed during the inspection, eyes had to adjust to the room to see children.

During the inspection the bathroom was observed, under the unlocked cabinet a bottle of Draino was seen within reach of children.

During the inspection the outdoor play area was observed. During the observation a kitchen knife, mouse pesticide foam and a bottle of lighter fluid were seen on the play area. I also observed 3 bushes that had sharp briar vines on them.

During the inspection advertisements were seen playing on screen time used for active play.

During the inspection a few holes and chipped areas were noticed in the walls of the children's play area.

During the inspection 2 of the 4 children's records were still missing the required immunization records or were missing some immunizations.

During the inspection a trash can in the children's bathroom was observed to have a used diaper inside in reach of the children.

During the inspection the operation policy was reviewed. The policy was missing the emergency preparedness plan and information for parents to contact the local licensing office/Abuse Neglect Hotline and access the HHSC website. The policy was also missing how parents can access Minimum Standards online.

During the inspection a bottle of flea shampoo and leather cleaner were observed in the bathroom cabinet without a lock in reach of the children. Multiple outlets were also observed to be uncovered.

During the inspection a few holes and chipped areas were noticed in the walls of the children's play area.

The sign-in and sign-out log for the past 3 months was not available for review during the inspection.

During the inspection the documentation for the emergency drills, smoke detector and fire extinguisher were not available.

During the inspection 4 children's records were reviewed and 3 were found to be missing required documentation or were outdated.

The end of nap time was observed during the inspection. The lighting in the nap room was dark enough that no children could be seen.

During the inspection a trash bag with old food was observed within reach of children. The children were using the trash bag to throw away items.

During the inspection an activity plan could not be observed.

During the inspection two bottles of alcohol were in reach of the children. The bottles had been there for more than a day. There was also a can of Rustoleum and lawnmower, and a propane tank outside in the area children use.

A statement of health and vaccination for the animals could not be viewed during the inspection.

During the inspection no fire extinguisher was present.

The operation did not have a carbon monoxide detector in the building.

During the inspection a flat screen televsion over 20 inches was observed not mounted or anchored. Children sat right under the television.

During the inspection the some walls were observed to have cracks and was broken in some parts exposing drywall. I also observed broken tiles in an area that children frequent that was broken.

The operation did not have a smoke detector during the inspection.
