Health inspection expired on 11/7

Based on interviews it was determined that a caregiver used inappropriate discipline on a child in care.

Operation did not notify Child Care Regulation within the required timeframe.

Operation does not have any diaper changing stations.

When a child soils themselves, they are sent to the nurse's office to wait for their parent to pick them up or to change them.

There was not fire drill documentation for the month of May. This was corrected at inspection when operation conducted a fire drill during this inspection. Drill was documented.

Upon reviewing personnel records, one staff's file did not have a Licensing Affidavit.

Upon reviewing personnel records, there was no documentation to verify that one staff is First Aid Certified.

I observed a tv on a rolling cart. TV was not anchored.

A child was left unsupervised outside by herself.

Staff attempted one call to parent following a situation that placed their child at risk. Parent did not answer. Staff did not make any other attempts to parents' work number or emergency contact. Parent was notified almost 40 minutes later about the incident when parent arrived to pick up their child.

There was no documentation for the months of December and January.

Upon reviewing children's records, several children were missing information: Healthcare information, admission date, Emergency Care Authorization, Food Allergy Information, Operational Policies Information and Healthcare Provider Statement.

Upon reviewing personnel records, director did not have a copy of her file/required information available.

Menu's were observed and were not dated.

Cots used by children to nap are not labeled.

Some rooms had outlets that were uncovered.

There was no documentation of drills accessible for review.

A first aid kit that was in a classroom was in a drawer that was unlocked, therefore making it easily accessible to children and was within their reach.

One employee who has a pending status was present.

Operation failed to notify Licensing of a reportable illness within the required timeframe.

Cots used by children to nap are not labeled.

3 staff who have direct access to children do not have background checks completed.

Menu's were observed and were not dated.

An employee who has direct access to children in care has an Ineligible status.

A weathered wooden structure was lying in the outdoor area where the children play. The structure has rusty nails. Also observed was a pile of cable that was also lying in the outdoor area where children play. I also observed a Lysol spray can disinfectant under an unlocked cabinet door in one of the classrooms.

A child was observed going from a water fountain that is near their classroom, back to the classroom. Another child was observed coming from a bathroom back to his classroom. Botth children were unsupervised.

Outlets in 3 rooms were observed uncovered. This was corrected at inspection when they were covered by staff.

Menu's were observed. Menu substitutions were made and not documented..

Drills are not being documented.

Surfacing is not the required depth under a large piece of outdoor climbing equipment.

Operation is providing services to children that are 3 years of age. Services are provided from 7:30am till 11:05am, Monday through Friday. Services began on August 10, 2021.
