Fire inspection is expired and has not been renewed.

The liability insurance has expired.

It's been over a year that the Children's Products form was signed. Date of last signature and date was 1/9/24.

Operation failed to complete mandatory report by the due date.

This standard was found to be deficient as part of a DFPS investigation.

It was determined that a staff did not demonstrate good judgement when staff gave a bottle to a child and did not check the temperature of the milk prior to giving it to the child. The child received 2nd degree burns as a result.

An inspection of the outdoor play area was not done on this day and a child found a knife/box cutter, on the playground.

I observed two sand boxes, one of them had cat/animal feces in it. Neither sand box had a cover.

There was no documentation of a fire drill for the months of April and May.

Operation did not document the substitution on the menu. Children were fed broccoli and cauliflower as a side, but menu stated that corn was going to be served.

The Children's Products Certification was not was recertifiied. The last date it was signed and dated was 1/22.

Allergy plan was not signed by parent and did not have symptoms to look out for and what to do in case of symptoms.

Upon reviewing children's records, 6 out of 10 children's records evaluated were missing required information. Information missing: address for emergency contacts, signatures for the special care statements, acknowledging if a child has a food allergy plan, acknowledging if parent received the operational policies, and signing off or acknowledging the healthcare provider statement.
