According to the information collected throughout the investigation, it was concluded that an infant was given the incorrect bottle due to the bottles not being properly labeled.

Two caregivers were observed with their personal iPads how while actively supervising children alone. One caregiver was observed during homework on her personal iPad while supervising children.

During the follow-up inspection, two caregivers did not know the correct number of children they were caring for.

During inspection an office worker did not follow the operation policy for release of children when aiding a unknown person to a classroom for pick up.

Based on information gathered during a DFPS investigation, it was determined that a child was left unsupervised on the playground for an extended period of time.

Based on information gathered during a DFPS investigation, it was determined that caregivers did not use good judgment when failing to properly conduct face to name checks during transition time.

Based on the DFPS Investigation, it was determined that a school age child was able to grab pepper spray that was attached to a caregiver's key chain.

Three infants were observed to be asleep in a restrictive device.

Based on evidence obtained, it was determined that a caregiver gave a child food that was listed on their food allergy plan.

The operation did not have the required posting up for a child with a medically diagnosed food allergy.
