The allergy list was not posted in the kitchen where food is prepared. This was corrected when the director copied the lists from the classrooms and placed them in the kitchen.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The handwashing sink in the infant room was not positioned in a way that it was being properly utilized. This was corrected when the director repositioned the sink and instructed the caregiver on proper use of the sink.

There was sufficient evidence to suggest that the list of employees had not been updated recently. This was corrected when the director corrected and reposted the list while CCR observed.

Two classrooms did not have posted written daily plans.

Two of seven children's records did not have complete or up to date health statements.

Two of seven children's records did not have updated immunization records.

There was sufficient evidence to support that multiple employees had a reasonable cause to suspect abuse and did not report to CPS.

The fire extinguisher in the van was observed expired, it was last serviced in May of 2021.

There were two areas of standing bodies of water observed on the playground accessible to children.

Children were observed during the inspection eating snack before handwashing. This was corrected during the inspection when the children were observed washing their hands.

An 18 month old child was observed having screen time during the inspection. This was corrected during the inspection when the television was observed being turned off and the children were transitioned to another room for naptime.
