The operation had not had a fire inspection completed by the local fire department in the past 12 months. The last inspection was March of 2024.

A caregiver acknowledged that they had not graduated high school on the personnel information record. The director will be speaking with the employee to see if this was an error.

An annual fire inspection had not been completed since April 2022. The director called the Allen Fire Marshal to schedule during the inspection.

A play structure was installed on the school age playground that was manufactured for residential use.

Two infants younger than 12 months were observed in cribs with loose bedding equipment such as blankets and bobby pillows. One infant was resting and the other infant was asleep. The caregiver corrected the deficiency during the inspection by removing the loose bedding equipment out of the cribs.

An infant was observed in a walker. The deficiency was corrected during the inspection when the infant was removed out of the equipment by the caregiver. The walker was removed out the classroom by management.

The activity schedules were not posted in the classroom that are utilized by children. The deficiency was corrected during the inspection when management posted the activity schedules in the classrooms that are utilized by children.

5 children were not supervised appropriately. The children were left alone napping in a separate room.

Three groups of children were supervised by 3 caregiver during nap time. There was not an additional staff or caregiver present upon arriving to the operation. The deficiency was corrected during the inspection when an additional staff member arrived.
