The assistant caregiver was lacking 10 hours of required annual training.

Information concerning 'Parent's Rights' was given solely to one child. The operational policies do not include this information.

Two children lacked the information regarding of the child s physician or an emergency-carefacility .

The primary caregiver was missing the following training: Emergency preparedness, communicable diseases, food allergies, understanding building and physical premises safety, handling and storing of hazardous materials.

The backyard fence is in a state of disrepair, with some pickets broken and rusty nails protruding from the wooden structure.

While the primary caregiver was abroad, the substitute caregiver who was present during the inspection encountered difficulty in accessing her own file, which contained important documents such as her Orientation, CPR/First-Aid Training, and Affidavit for Applicants for Employment. Unfortunately, the substitute caregiver's file was not available for review at the time of the inspection.

Upon review, the operation had an incomplete emergency preparedness plan missing required information.

The operational policies were missing the following items: Promotion of indoor and outdoor physical. Your policies must include: (A)The duration of physical activity at your home, both indoor and outdoor [Medium]; (B)The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (C) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor time.
