Three caregiver files are missing an Employment Affidavit Form 2985 and Form 2912.

A caregiver supervising children under 24 months does not have the following training: Recognizing and preventing shaken baby syndrome and abusive head trauma; Understanding and using safe sleep practices and preventing sudden infant death syndrome(SIDS); and Understanding early childhood brain development.

A caregiver's file does not have proof of education.

Observed a torn mat that is use for diaper changing.

Observed that classooom # 1 and classsroom # 5 are not set up for children to use during business hours.

There is no supporting documentation that a fire drill was conducted in September and October 2023.

Two staff members are not current with transportation training.

There are 2 classrooms inside the building which are licensed and are now being used as storage rooms and not ready for children. (Pictures on file.)

There were 6 infants in care however 1 caregiver or the other would walk out of the classroom and leave the other alone with the 6 infants. (Both caregivers finally stayed in the classroom)

There were 2 infants who were not asleep on their backs. One was asleep in a swing and another was asleep in a bouncer which was inside a crib. (Pictures on file)

There were napping mats and blankets all piled up on a table in the preschool room. (pictures on file)

There were 2 infants who were sleeping and were covered with blankets. (NOTE: Corrected during inspection as the blankets were removed.)(Pictures on file)

Out of the records reviewed, two had incomplete information for the person to contact in case of an emergency. (NOTE: Corrected during inspection as parent was contacted for missing information.)

There was a children's admission form that was not signed by the parent.

Facility has not designated a director.
