At the time of review two out of six children's records did not have current immunization records.

The staff did not have the required trainings done at the time of review.

One of the two transportation vehicles did not contain a First Aid Kit as required. NOTE: This was corrected at inspection when the director placed an extra First Aid Kit into the vehicle.

The binders for each of the transportation vehicles did not contain signed medical authorization forms for the children that are transported. One of the two transportation vehicles did not have emergency contact information for children transported in the vehicle.

The operational policies did not include the promotion of indoor/outdoor physical activities.

Based upon an observation and information gathered through an interview the lack of supervision and knowledge of the incident which occurred in the toddler classroom that resulted in a child sustaining an injury was determined to have occurred. NOTE: Since the incident the toddler caregiver was terminated.

A toddler was observed on the changing table as the caregiver did not have a hand on the child with their back facing the child.

One of the six children did not have immunization shot records for review.

In the preschool library center, a TV was observed on a table leaning against the wall not mounted. This was corrected at inspection when director removed the TV and placed it in a locked closet.

Operation did not have the annual fire inspection posted for the current year.
