The toddler classroom was out of ratio there were 12 children with 1 caregiver. NOTE: This was corrected at inspection when a second caregiver was placed in the toddler classroom making the classroom come to ratio.

Staff files were reviewed and 2 out of the 4 staff files were missing trainings in the following training topics. * Preventing and responding to emergencies due to food or an allergic reaction. * Preventing and controlling the spread of communicable diseases, including immunizations. * Administering medication. * Handling, storing, and disposing of hazardous materials including compliance.

In the infant rooms one of the mats was not labeled with the childs name. NOTE: This was corrected at inspection when caregiver labeled the mat.

During inspection it was observed the ATTENTION PARENTS required posting was not posted. NOTE: This was corrected at inspection when director posted the ATTENTION PARENTS letter in the entrance of the center.

During inspection it was observed the most recent fire inspection was not posted.

An employee without a high school diploma/GED was left in charge of supervising a classroom. No other caregivers were present in the room. Note: This was corrected at inspection when a qualified caregiver stepped into the room.

An employee was present at the operation and caring for children without background check eligibility notice.
