A child was observed to have an expired epi-pen, that was no longer in use. The medication was taken out of the kit and returned to the parent.

The parent of a child with a diagnosed allergy had not signed the allergy plan.

During the monitoring inspection it was found that there was a child at the operation with an allergy to peanuts, requiring an emergency plan, and one was not able to be located.

During the inspection one of the two staff files evaluated did not have documented proof of orientation being complete.

During the inspection one of the two staff files evaluated did not have documentation that preservice was complete prior to have unsupervised care with children in care.

During the inspection one caregiver file had 6 out of the 15 required annual training hours.

During the inspection one caregiver was in the care of 32 children ages 5-8 years old

During the inspection a caregiver counted how many children were in care and stated 41. There were 43 children in the gym and one in the restroom during this time.

During the inspection two caregivers were in the care of 43 ages 5-12 years old. This was corrected during the inspection by a third caregiver arriving.
