Four out of ten staff records had CPR/First Aid training certifications which were not in person training.

Three infants who are exlusively bottle fed do not have infatn feeding schedules updated within the past 30 days.

This standard was evaluated during the investigation and was found to be deficient. A caregiver forced a child to sleep by repeatedly pushing the child's head down on a nap mat, covering the child's head with a blanket, and placing their legs on the child's back during nap time.

During the inspection a 9 month old infant was observed asleep with a blanket in the crib. This was corrected when the blanket was removed from the crib during the inspection.

During the inspection there were outdoor play materials which were broken. Two slides which had been broken off from a play stucture were on the playground, three outdoor play tables had broken pieces, and a wooden playhouse had broken doors with sharp edges and staples coming from it which were sharp.

During the investigation inspection a staff member was left in charge while conditions state that they are not allowed to be in charge of the operation. This was corrected during the inspection when the owners and interim director arrived during the inspection.

This standard was evaluated during the investigation and was found to be deficient. During interviews staff members stated that a long phone cord was in reach of children. This was corrected prior to the inspection.

This standard was evaluated during the investigation and was found to be deficient. The operation did not notify CCR of water damage sustained in a classroom which rendered the classroom unsafe or unsanitary.

This standard was tasked over for this investigation in found to be deficient. It was determined that a child was left unattended in his classroom during recess time. The child was found in his classroom by the center director while his classmates were outdoors. The operation now has a name to face check system and transition log that has to be completed throughout the day to ensure that each child is present. This logging system and transitions were observed during the inspection time. There are also new caregivers present in the named classroom, additional supervision training will take place, and children were properly supervised at the time.
